KPA Charitable Donation Form
Kentucky Performing Arts' mission is to connect and inspire through the arts. As a 501(c)(3) non-profit organization, KPA has a limited amount of gift certificates to donate to qualifying non-profit organizations for use in their fundraising efforts (silent auctions, raffles, etc.).
Please complete and submit this form no more than 4-6 weeks before your event. A member of our development staff will reach out to the contact person within 10 business days of receipt, if your request is approved.
Please note:
*A limited number of gift certificates are available each quarter. Requests will be reviewed in the order they are received. Please apply only once per organization.
*Only non-profit organizations are eligible for consideration - a recent IRS 990 form must be uploaded with this form.
*A member of the KPA development office team will email the contact person listed on the form within ten business days if request is approved.
